District Forms
Athletic physical - Every year students (grades 7-12) must have a physical examination before participation in school sponsored athletics. Completed athletic physicals are kept on file in the athletic director's office.
Parent Athletic Emergency Form - This form must be completed and returned to your coach prior to practice and/or participation in a sport. (updated 11/7/11)
School Physical Form - School physicals are requested for students entering third, sixth and ninth grades. Completed forms should be submitted to the school nurse. (updated May 2009)
Asthma Consent Form - In order for a student to self-administer medication for asthma or any airway constricting disease, this form must be completed and returned to the school nurse.
Medication Permission Form - Physician orders and parental consent must be obtained before medications will be administered at school. Completed forms and medications should be given to the school nurse or the building secretary. All medications should be delivered to the school by an adult. (updated 02/10)

Certificate of Dental Screening Form - All children entering kindergarten, ninth grade, or transferring from an out of state school must have a dental screening. Kindergarten must have the screening completed between 3-6 years old. Students entering high school or transferring must have the dental screening within one year prior to enrollment. Pre School Physical Form - Children entering preschool need a physical examination done within the past year. (updated 05/20/10)
Concussion Form - Required of students in grades 7-12 prior to participation in any extra curricular interscholastic activity. (updated 11/7/11)
